Prognostic significance of micrometastasis and isolated tumor cells in the sentinel lymph node

Maaskant-Braat and colleagues [1] contribute to our understanding of sentinel node micrometastatic disease with this population-based study. Using data from the Eindhoven database of 6,803 women with breast cancer during a 10-year period from 1996 to 2006, the authors find no statistically significant difference in survival between patients with sentinel node micrometastatic disease (n = 451) or isolated tumor cells (n = 126) compared to those patients with tumor-free sentinel nodes. This lack of survival difference persisted even when adjusting for age, tumor size, grade, and adjuvant systemic therapy. Given these results, the authors postulate that micrometastatic disease in the sentinel lymph node of itself should not be an indication for adjuvant systemic therapy.
