Randomized clinical trials and meta-analyses have clearly demonstrated that radiation after definitive surgery can improve local-regional control and consequently breast cancer survival. There has been some controversy, however, regarding when and in which patient populations more extensive radiation to the nodal regions is warranted. This review focuses on the current evidence supporting the use of radiation and the rationale for determining patients who will derive the most benefit.
