Prior to the availability of effective posttransplant immunoprophylaxis, outcomes following liver transplant for hepatitis B-related liver disease were very poor as a result of high rates of graft reinfection and accelerated graft failure. With the advent of hepatitis B immune globulin and nucleos(t)ide analogue therapies, reinfection rates and transplant outcomes have dramatically improved. Although current immunoprophylaxis treatments are highly effective in preventing reinfection, they are very costly and cumbersome to administer. The goal of evolving strategies is to minimize the dose and duration of hepatitis B immune globulin treatment without a loss in efficacy. As the number of effective, direct acting antiviral therapies has increased over the past several years, the possibility of hepatitis B immune globulin-free regimens now exists. Limited, but encouraging, data exist to support hepatitis B immune globulin-minimizing regimens. Research is needed involving novel hepatitis B immune globulin-free strategies.
