Description
A 14-year-old female inpatient with systemic lupus erythematosus, renal failure, ascites, neutropenia, and thrombocytopenia was noted to have an abrupt onset abnormal rhythm on telemetry. The rhythm was characterized by shifting P-wave and QRS axes without QRS widening, S-T changes, or tachycardia (Fig. 1). The pattern seemed to be respirophasic. Despite the rhythm alteration, she remained clinically stable and asymptomatic.
