Sirs, Acute renal failure (ARF) affects 8–24% of babies admitting to neonatal intensive care units [1]. Dialysis or hemofiltration is the treatment of choice, even in premature babies. The minimum hemodialysis circuit volume is large in proportion to a baby’s blood volume. Therefore, peritoneal dialysis is preferred, even in the very low birth weight infants, but in some situations like peritonitis or after abdominal surgery, peritoneal dialysis is impossible. We report on exchange transfusion as rescue therapy for blood purification in an extremely low birth weight premature infant in whom peritoneal and hemodialysis were impossible.
